Public Housing Applications are available by downloading the PDF file from the link below. ALL completed applications must be submitted IN PERSON.
DO NOT MAIL or EMAIL your application back to the housing authority.
You must make an appointment with the Intake Coordinator to submit your application. All persons 18 and older in the household must be present to submit an application.

Please complete your Public Housing Application and contact Ms. Katie Howard at (573) 443-2556, ext. 1111 or khoward@columbiaha.com to schedule an intake appointment. Applications are accepted by appointment only. All adult members of the household must attend the intake appointment. Please review the Public Housing Intake Process to ensure timely processing of your housing application.
